Save the forms to your computer by selecting "Save As" in the file menu. Once the documents are saved to your hard drive, information can be filled in, saved, and emailed to info.DrKarena@verizon.net.
The forms can be filled in online and printed out as well. Documents can be faxed to (212) 759-3170.
